New Client Information

We would love to have your pet as a new patient at the Cheat Lake Animal Hospital! 

We are always accepting new clients and patients.  As a new client to our facility, you are able to request appointments, boarding reservations, and grooming appointments.

 Please be aware that we will need copies of any previous medical history including vaccination records.  Please have these records faxed to 304-594-1911 or bring them with you on your first visit to our hospital.
